- Pampers Diapers box size 3, 4, 5 or 6

You must buy them in-store @ Walmart, between July 15th and December 15th, 2008. Then fill in your personal details on the freebie!
It does not say “one per household” as some of these offers do, so I’d highly recommend stocking up on some Pampers products and reaping the rewards!
