The items must be on the same cash register receipt, and must be dated between May 2, 2016 and May 30, 2016. You must complete the online form prior to June 30, 2016.
You can even opt to receive an e-Gift card (via email) instead of a physical card so you will get the $10 reward more quickly. I always put my gift card on my Tim's phone app anyway so the e-Gift Card works out great!
