This is a more adult oriented post: Play Time by Durex is offering a free sample of their personal lubricant. They are also offering a printable coupon for $1.00 off their personal lubricant, in case you really, really like it!
In either case, just fill out the form and submit. 
I tested both offers and the coupon showed up a few seconds after submitting my info, and the application for the freebie processed fine.
Here are a few words from Play Time by Durex regarding the freebie:
The lubricant sample you receive will be ramdonly selected among Play™ More, Play™ Warmer and Play™ Tingling. Must be a U.S. or Canadian resident to receive a sample. Please allow 12-15 weeks for delivery. Limit 1 per customer.

if you look at the fine print, the coupon for a buck off expired in april of 2006.
